Abstract: FORMS WH-3 AND WH-35P ARE USED TO OBTAIN INFORMATION FROM INDIVIDUALS ABOUT ALLEGED VIOLATIONS OF VARIOUS LAWS ENFORCED BY THE WAGE AND HOUR DIVISION. IT IS ALSO USED AS A SCREENING DEVICE TO DETERMINE WHETHER THE DIVISION HAS JURISDICTION IN HANDLING THE ALLEGED VIOLATIONS.
