Approved for use through 12/94 under the condition that AHCPR agrees to present the results of this study as information specific to the States of California and Iowa, and does not attempt to generalize thes results in policy making to other States or regions of the country. Though this study's design may control for variances within institutio and specific States, it does not address differences in medical practi between States. For instance, Medicare PRO policies historically have been premised upon the assumption that perceptions of appropriate care vary significantly by locality and region. Therefore, OMB believ that this study's results should be used as the basis for a more comprehensive, national effort. In addition, in its final report AHCP should analyze the non response characteristics for those respondents who do not have home phones. In its supporting statement, AHCPR attempts to predict the characteristics of these respondents, and asserts that initial contact and follow-up will minimize any problems. OMB believes that nonrespondents without phones may include the most vulnerable populations. Finally, AHCPR should attempt to understand the extent to which patient outcomes are influenced by facility experience levels in performing each of the procedures. For example, Medicare transplant coverage policy already assumes that facilities performing procedures more frequently may provide enhanced quality of care. AHCPR should submit this study's final report to OMB.

THIS RESEARCH PROJECT WILL STUDY THE VARIATIONS AND CORRELATES OF SPECIFIC DIAGNOSTIC AND TREATMENT PROCEDURES USED IN MANAGING LABOR AN DELIVERY. OF PARTICULAR INTEREST IS THE DECISION WHETHER OR NOT TO DELIVER BY CESAREAN SECTION, INCLUDING THE OUTCOMES OF THAT DECISION. THE PROJECT WILL YIELD INFORMATION TO HELP IMPROVE MEDICAL CARE IN CHILDBIRTH, IMPROVE MATERNAL AND INFANT HEALTH, AND REDUCE COSTS.
