Terms of Clearance: Approved for use through 1/95 under the condition that ORD adds questions regarding the percentage of patients or services falling within Health Maintenance Organization arrangements. Such a question should be inserted into the "Facility Overview" of each of the three surveys, i.e. hospital outpatient, physician office, and the ambulator surgical center surveys. In addition, question 8, section A on payor mix should either be amended or supplemented to capture each facility' payor mix for patients.
