As agreed to by the Agency, HCFA will make the changes outlined in
the April 30th memorandum from Project Hope. These changes were
made to shorten the questionnaire, in an attempt to increase
anticipated response rates by physicians.

THIS MAIL SURVEY OF PARTICIPATING AND
NONPARTICIPATING MAC PROVIDERS WILL EXAMINE THE EXTENT TO WHICH
PRIMARY CARE PHYSICIANS ARE WILLING T PARTICIPATE IN MAC AND HOW
EFFECTIVELY THEY MANAGE THE CARE. THE SURVEY WILL BE CONDUCTED IN
TWO WAVES.
