Terms of Clearance: This information collection is approved through 9-94 under the following conditions: As agreed to by SSA, the agency will create a a new question 4D to guage what percentage of those that potentially qualify for SSI were referred to SSA for processing an SSI application. The old question 4D will be changed to 4E to solicit the names and numbers of those referred. As indicated by SSA, the agency conducts followup on these individuals to see if they qualified for benefits.
