Abstract: THE FORM WAS DEVELOPED FOR USE BY LOCAL GOVERNMENTS IN CONJUNCTION WIT THE COMMUNITY DISASTER LOAN PROGRAM (SECTION 417, P.L. 93-288 AS AMENDED). THE FORM IS UTILIZED TO REQUEST CANCELLATION OF A COMMUNITY DISASTER LOAN IN ACCORDANCE WITH THE PROVISIONS OF THE LAW AND REGULATION PERTAINING TO COMMUNITY DISASTER LOANS.
