AGREEMENT STATE LICENSEES, AS A MATTER
OF COMPATIBILITY, ARE REQUIRED REPORT MISADMINISTRATION AND EVENTS
INVOLVING PERSONNEL OVEREXPOSURES LOST, STOLEN, ABANDONED, OR
DAMAGED RADIOACTIVE MATERIAL TO THEIR RESPECTIVE AGREEMENT STATES.
NRC IS REQUESTING VOLUNTARY SUBMITTAL OF THIS DATA TO NRC IN A
UNIFORM SUMMARY FORMAT.
