MAIL, TELEPHONE, OR IN PERSON CONCERNING BENEFITS THEY HAVE APPLIED
FOR, WERE DENIED, OR ARE RECEIVING UNDER PROVISIONS OF THE RAILROAD
RETIREMENT AND RAILROAD UNEMPLOYMENT INSURANCE ACTS. THE COLLECTION
WILL OBTAIN INFORMATION FOR DETERMINING THE LEVEL OF SATISFACTION
WITH THE SERVICE PROVIDED BY
