Abstract: S MEDICATIONS, PRESCRIBED, PHYSICIAN' THE INFORMATION COLLECTED BY THE HA-4632 IS USED TO COMPILE A CURRENT LIST OF THE MEDICATIONS USED BY A CLAIMANT. THE LIST IS PROVIDED TO A ADMINISTRATIVE LAW JUDGE (ALJ) WHO IS CONSIDERING THE DISABILITY ASPEC OF THE CLAIM. THE AFFECTED PUBLIC CONSISTS OF CLAIMANTS FOR DISABILITY BENEFITS WHO HAVE REQUESTED A HEARING BEFORE AN ALJ.
