This information collection is approved through 8-94 under the followi conditions: As agreed to by ACF, the Agency will compare the findings of this evaluation to the aggregate data reported on the Program Information Report. This will include a comparison of information on the health status of the child, including immunization records as reported by the parents versus what the Center has in its records. The accuracy of reported data will become increasingly important as the program moves to outcome-based evaluation which will likely rely heavily on record-keeping. The Agency will also make changes in the parent interview to 1)# 5, change Multiracial to "Other race"; 2) eliminate question22; 3)Rephrase # 51a to read "Did you make any changes in the following areas? For the Health Coordinator Interview, ACF will 1) Modify # 24 to make these value-free statements, i.e. a. Frequency of Meetings. These changes should also be made in the other coordinator interviews where necessary. Finally, ACF will delete the Education Coordinator Interview because it is duplicative of information already collected through the other interviews.

THIS STUDY WILL PROVIDE A DESCRIPTION OF THE HEAD START HEALTH COMPONE ACROSS FOUR DOMAINS: MEDICAL, DENTAL, NUTRITION, AND MENTHAL HEALTH. RECORD REVIEW, PARENT INTERVIEWS, AND HEAD START STAFF INTERVIEWS WILL BE CONDUCTED AT 80 HEAD START CENTERS NATIONWIDE TO CREATE A NATIONAL PROFILE OF HEALTH SERVICES AND BARRIERS FAMILIES AND CENTERS FACE IN ACCESSING HEALTH CARE.
