collection is approved through 6-95 under the following conditions:
OMB discourages the use of visual identificatio of race and
ethnicity status of individuals because of the method's
vulnerability to inaccuracy. Instead, Agencies are encouraged to
offer individuals the opportunity to complete a self identifying
form which is submitted separately from the application form. In
the June 9, 1994 Federal Register, OMB solicited comments on this
and other race/ethnicity data collection issues. Anticipating the
results of this Notice, FNS will submit a plan to convert to a a
self-identifying system upon resubmission of this information
collection.

THE FNS-191 FORM IS USED BY WIC AND
CSFP LOCAL AGENCIES TO COLLECT RACIAL/ETHNIC DATA ON EACH
PARTICIPANT FOR ONE MONTH OF THE YEAR. THE COMBINED COLLECTION OF
RACIAL/ETHNIC DATA IS USED TO MONITOR THE WIC, CSFP, FSP, AND FDPIR
LOCAL AGENCIES' COMPLIANCE WITH THE CIVIL RIGHTS ACT OF
1964.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
