The Reader's Survey (indianapolis) dated August 3rd is approv as submitted. 8/15/94. Approved with the guideline submitted 8/10. IRS should subm report to OMB at the completion of the survey, including the number of Remarks added, eff. 10/12/94: PC:V 94-008 is approved as submitted. Project PC:V 94-009-G is approved with the conditions that: 1) any changes proposed after the test period will be submitted to OMB for review, 2) a report of the test will be submitted upon its completion, and 3) a report of the project will be submitted including the response rate at the completion. PC:V 94-004 is approved as submitted.
