Abstract: THE INFORMATION COLLECTED BY THE USE OF FORM SSA-4513 IS USED TO BUDGE AND ACCOUNT FOR EXPENDITURES BY THE STATE DISABILITY DETERMINATION SERVICES (DDS) WHICH ARE INCURRED WHILE IMPLEMENTING CERTAIN PROVISION OF THE DISABILITY PORTION OF THE SOCIAL SECURITY ACT.
