Abstract: THE INFORMATION COLLECTED BY THIS FORM WILL BE USED IN CONJUNCTION WIT THAT OBTAINED USING THE SSA-870 AND WILL HELP DETERMINE THE FUNDS EACH STATE DISABILITY DETERMINATION SERVICES AGENCY (DDS) NEEDS TO MAKE DISABILITY DETERMINATIONS FOR SSA FOR THE COMING YEAR.
