Abstract: INFORMATION ON C CORPORATION SHAREHOLDERS IS REQUIRED TO COMPLY WITH T WOMEN'S BUSINESS OWNERSHIP ACT OF 1988. AN EXECUTIVE OFFICER WILL COMPLETE THE FORM FOR THE C CORPORATION.
