one-year clearance, since these forms may require revisions
following publication of interim final regulations for the DUA
program; this extension will also be given to the ETA form 90-2.
Last, we note that DOL attributes a printing error to the
establishment of a 6/95 expiration date on the previous approval
for this form; OMB's previous notice of action set an expiration
date of 9/94. DOL shall review all forms carefully to ensure that
the printed expiration date matches that provided on the NOA.

PROVISION OF THIS "BENEFIT ASSISTANCE, UNEMPLOYMENT, COMPENSATION
CLAIMS, AND FINANCIAL MANAGEMENT."
