THESE DATA WILL BE USED TO MAINTAIN AND IMPROVE HEALTH CARE AND WILL BE OBTAINED FROM INSURERS, LICENSURE BOARDS, PEER REVIEW COMMITTEES, HOSPITALS, AND OTHER PROVIDERS.
