IN CERTAIN CASES, THIS INFORMATION MUST BE FILED ON MAGNETIC MEDIA. FORMS 4802 AND 4804 ARE USED TO PROVIDE A SIGNATURE AND BALANCING TOTALS FOR MAGNETIC MEDIA FILERS OF INFORMATION RETURNS.
