Terms of Clearance: Approved as amended by ED's 9/21/95 memorandum to OMB. In addition, ED shall meet the following conditions: -- ED shall revise its schedules to ensure that this form can be used by States to satisfy their annual application requirements, thus eliminating the need for a separate requirement. If this is not feasible, then approval is granted for collection of this information only on a biennial basis. -- Regardless of frequency, this form will be allowed to satisfy the requirement for a biennial evaluation of the early intervention component. The instructions shall be revised to reflect this, adding any items required to be included in the biennial evaluation under the statute and regulation (34 CFR 693.52).
