NARA will revise
Form 13075 "Questionnaire about Military Service" as double-sided
with a larger, more readable typeface. NARA will ensure the OMB #
and burden statement appear on the face of both of the forms, and
that the statement removes reference to sending comments to OMB.
The Privacy Act statement must be modified to include the major
routine uses of the information, if any, in accordance with 5 USC
552(e)(3)(C), and OMB will work with NARA/VA/DOD to ensure
compliance with this provision. The form may include notice that
other agencies may include addenda to the routine uses, but any
such addenda must be approved by OMB under this OMB control #.
Copies of the final printed forms should be sent to OMB for
inclusion in the public docket.

The information collection is
information that must be provided when requesting information from
or copies of documents from a military service record stored at the
National Personnel Records Center to allow NPRC to locate the
record and respond to the request. Respondents are veterans, their
authorized representatives, state and local governments, and
businesses.
