collection is approved for use through 6/96 under the following
term of clearance: 1. The agency will report to OMB on the results
of the pilot test prior to receiving approval to undertake a full
study. This report should include feasibility of collecting
information about children from adult respondents; success of using
a list assisted, random digit dialing survey; a discussion of the
health indicators demonstrated in the participating localities; and
an evaluation of the cognitive design of the instrument.

This project will pilot test a
telephone survey questionnaire for use in collecting comparable
health behavior information at the local health department or
community level. The pilot will assess the costs of using a
list-assisted random digit dialing sample design, evaluate the
collection of information for children, and monitor the utility of
the data to participating local health departments.
