The FCC shall remove OMB's address from the Public Reporting Burden statement on page 1 of the instructions. The FCC shall also closely review the draft form for consistency and accuracy. For example page 3, Section I, item 7 requests a "contact number" while item 8 requests a "contract number." It would appear that both should read "point of contact telephone number at the sponsoring agency" or something similar.
