In the interest of allowing DoD to pursue Gulf War research expeditiously, OMB approves use of this survey effort through 3/96 although a significant number of fundamental concerns remain that must be resolved fully with OMB prior to fielding this instrument. OMB understands that this submission covers only Phase I of the survey effort. DoD must continue its dialogue, however, on its decision to conduct this survey in two phases and postpone the collection of risk factor, exposure, and sensitive outcome data until Phase II. The Phase II design necessitates collection of name, address, and phone number information in addition to social security number. Collection of this personal identification information may affect the validity of certain responses or contribute to item nonresponse. DoD shall present OMB with additional information supporting its decision to design this effort in two parts and to split the questions. In addition DoD shall provide OMB with additional information concerning its validation plans and privacy protections. DoD must present its privacy act statement and intended routine uses for OMB concurrence. Finally, DoD must further discuss the differences between this effort and the VA national prevalence survey and the CDC Iowa study. OMB may ask that DoD change its sampling metho- dology and questions so the results of this effort can be compared with these other surveys. DoD must receive final OMB concurrence on its survey instrument and sampling methodology before fielding this instrument. These conditions are designed to ensure that DoD's effort meets the PRA standards of maximum usefulness with minimum burden.

The Department of Defense needs information on morbidity and adverse reproductive outcomes among Gulf War veterans and associated risk factors. The information will be used to determine whether participation in the Gulf War Theater of Operations had any effect on the health status of subjects. All active duty military, reservists, and National Guard on duty anytime between August 1990 and August 1991 will be eligible for study.
