Abstract: These forms collect eligibility information from prospective CHAMPVA beneficiaries, claim payment/reimbursement of covered medical expenses, obtain interim and systematic OHI information, correctly coordinate benefits among all liable parties, prevent or detect fraud, and are used in the recovery of costs associated with medical services related to an injury/illness caused by a third party.
