SF-2802B MUST ALSO BE COMPLETE IF THERE ARE SPOUSE(S) OR FORMER SPOUSE(S) WHO MUST BE NOTIFIED OF THE EMPLOYEE'S INTENT TO TAKE A REFUND. RI-36-7 IS NEEDED WHEN THE SF-2802 IS INCOMPLETE AS TO THE APPLICANT'S MARITAL STATUS.
