INFORMATION WHICH WILL INCLUDE OTHER GOVERNMENT ASSISTANCE BEING
REQUESTED, NAMES AND FINANCIAL INTERESTS OF ALL INTERESTED PARTIES,
AND A REPORT OF EXPECTED SOURCES AND USES OF FUNDS. A $200,000
THRESHOLD APPLIES TO THIS DISCLOSURE REQUIREMENT.
