This information
collection is approved for two years subject to the following terms
of clearance: 1. The agency shall reduce the scope of the study to
a pilot study in which no more than 150 individuals are surveyed.
2. There shall be no remuneration in the survey. 3. Upon submission
of the follow-up study, the agency shall submit a plan that
describes utilization of a sample generaliz- able to the
population; determine whether the Kerachsky-Mallar remuneration
technique (for hard to locate respondents) can be adapted to the
out-hospital interview strategy; and evaluate the feasibility of a
"capture-recapture" survey model for the follow- up study. 4. The
agency shall submit the revised instruments as appendices to this
package. 5. The agency shall significantly scale back the surveys
to a length of no more than 20 minutes per interview, in order to
increase the response rate.

The CDC proposes to conduct a
longitudinal study to determine the outcomes and use of services
following firearms injuries. Emphasis will be placed on level of
disability and barriers to receiving post acute services.
