Abstract: The information collected by SSA-2506 is used by SSA to evaluate the severity of mental impairments in adults who have filed a claim for disability benefits. The respondents will be State Disability Determination Agencies who are responsible for reviewing the beneficiaries' claims and report their findings to SSA.
