Terms of Clearance: This collection is approved as amended on the following conditions. First, one-year approval should provide ED enough time to significantly revise the form to focus more on performance and outcome information for individuals served by the program. As stated in OMB's memorandum of 1/17/96, OMB believes that the form should undergo a major overhaul and that plans to make the necessary revisions should begin now while ED awaits reauthorization. Second, ED agrees to note in the collection package that an electronic version of the form is available to grantees upon request.
