This information
collection is approved through 3-97 under the OMB/FCS agreement
articulated in the attached FCS memorandum. In addition, as agreed
to by FCS, the agency will separate the section of the overall
teachers questionnaire that solicits data on the teachers's
personal eating habits (16-20,22,23). FCS will reiterate that the
set of personal eating habits questions are optional. This
agreement also applies to the post-test and control group
questions. FCS will also ensure that adequate "firewalls" exist
between those individuals that are reimbursed by the school
district to implement the survey and any respondents participating
in the survey. Finally, FCS will be explicit in their instructions
to teachers, students and parents that this is a voluntary
survey.

Team Nutrition Pilot communities
promote healthy food choices through a school-centered nutrition
effort. The evaluation will assess impacts on student behavior and
motivation.
