Abstract: Document the real effects of earthquakes. This questionnaire is designed to collect information concerning the damage or degree of shaking from earthquakes. Data is used in mapping the extent of the shaking in term of intensity, input to building codes, and data for earthquake research. Primary respondents are local, state, and Federal employees (individual citizens do not receive questionnaires by mail--some voluntarily complete questionnaires)
