The Department of Defense provides
members of the Uniformed Services, their status as an active duty,
Reserve, retired member, or civilian, and as an authorization card
for Uniformed Services benefits. This information collection
supplies the necessary information to identify applicants' status
and eligibility. The DD Form 1172 collects information and is the
application used for issuance of the Uniformed Services
identification card.
