Terms of Clearance: In accordance with the Paperwork Reduction Act of 1995 and 5 CFR 1320, this collection must display the following statement on both forms and all future Federal Register notices: "An agency may not conduct or sponsor, and a person is not required to respond to, collection of information unless it displays a currently valid OMB control number." In addition, the form 3814-4 requires a complete Paperwork Reduction Act notice, as required in 5 CFR 1320.8(b)(3). The agency should submit to OMB an OMB form 83-C which describes the previously approved information collections which are now included in this collection.
