Terms of Clearance: The FCC must make the following changes and clear them with OMB before printing additional copies of this form: a) Where appropriate, modify the form to include the statement on the use of the Taxpayer Identification Number required by the Debt Collection Act, b) add a line to the instructions that tells filers what to provide in line 11 if they do not have a social security number, and c) modify the Privacy Act statement to more specifically cite the authority to collect this information. At a minimum, the FCC will redact the social security number from any filing placed on public display. Additionally, the FCC will ensure that all personal information (including date of birth) is protected from inappropriate or unnecessary release to the public. The FCC has permission to delete the expiration date provided a date of issue is printed on each form.
