The FCC shall
clarify the Privacy Act Statement by adding a specific citation of
the Telecommunications Act regarding the authority to collect this
information. The FCC shall also verify that this form does not need
to be modified to reflect the Debt Collection Act. Finally, the FCC
should verify the applicability of the penalties listed in Section
5 of the form for willful false statements as this collection is
used for obtaining FCC equipment authorizations rather than
licenses of broadcast stations.

A completed application, combined with
descriptive information, test data, and occasionally a test sample,
provides information to determine compliance of the subject
equipment to the FCC Rules, thereby controlling interference to
radio communications. This data may also be used to aid in
enforcement of the FCC Rules.
