Terms of Clearance: Approved; INS request to not display the expiration date on the form is approved. INS shall add the statement required under 1320.5 (b)(2)(i) when reprinting the form. Also, INS shall move the statement on the voluntary nature of the form to the first page in a more prominent location upon reprinting the form in order to better inform the respondent up front that completing the form is voluntary.
