submits to OMB: 1) an extensive crosswalk between the LSOA II and
other HHS surveys of the elderly popula- tion (i.e. the AHEAD,
NLTCS, and MCBS.); 2) results of the 50 person pretest as soon as
it has been completed and a summary of any changes that need to be
made in the instrument and survey process; and 3) an estimate of
the expected attrition in the 1994 SOA sample due to death or other
factors and a more detailed estmate of any attrition expected
during the various phases of the LSOA II.

The LSOA II is a second generation
survey designed to 1) replicate portions of the first LSOA in order
to track changes in aging and health over time, and 2) to provide a
mechanism for monitoring the impact of changes in Medicare and
Medicaid on the elderly. Data from the LSOA II will enable policy
makers to plan and evaluate public health prevention programs for
the elderly and prepare estimates of future burden and health care
costs. The respondent universe for the LSOA II data collection
consists of approximately 10,037 respondents, 70 years of age or
older at the time of the baseline interview in 1994.
On behalf of this Federal agency, I certify that
