This information
collection request has been improperly submitted because ATF is
requesting an extension without change of a form that ATF has
revised. OMB has extended by one month the expiration date of the
version of this form approved by OMB on March 9, 1994 to allow ATF
to resubmit this request. The submission should request an
emergency revision. ATF must justify the new questions and the
change in instructions in light of new legislation and recalculate
the burden hours accordingly. Upon OMB approval of the
resubmission, ATF will publish a pre-clearance notice in the
Federal Register that describes the revisions and requests public
comment. This will begin the process of obtaining three-year
approval of the revised form.

This form is used to determine the
eligibility (under GCA) of a person to receive a firearm from a
Federal firearms licensee. It is also used to establish the
identity of the buyer. The form is also used in law enforcement in
investigations/inspections to trace firearms or to confirm criminal
activity of persons violating the GCA.
