Approved for use through 06/2000 under the condition that NCHS understand that because of its low response rate, this effort cannot be used to infer satisfaction for the entire respondent universe or even a subgroup. In addition, prior to fielding this instrument, NCHS must submit to OMB a more formalized plan for follow-up with significant organizations or respondents with particular characteristics that would inform the evaluation of the U.S. Standard Certificates.

The overall objective of this information collection is to evaluate the 1989 revisions of the U.S. standard certificates of birth and death and the U.S. standard report of fetal death. The responses to the information collection will help determine what changes are needed in the current standard certificates. Respondents will include users and providers of vital statistics data.
