agreement. Additionally, the FCC should review the form for
compliance with the Debt Collection Act. If needed, the FCC may
revise the form to add the TIN information, provided the FCC
revises the privacy statement accordingly. OMB recommends
clarifying the payment requirements and procedures -- particularly
use of the FCC 159.
