ED will continue their efforts to put this form on the
Rehabilitation Services Administration bulletin board and will
update OMB on their efforts; ED will, at the next opportunity to
contact all the project directors of this program, collect e-mail
addresses from the project directors so that this form can in the
future be sent out over e-mail; ED will continue to ensure that the
information requested in this form is not duplicated in the Annual
Performance Reports; ED will update OMB if any regulatory changes
affecting the current indicators occurs prior to distribution of
this form under current clearance. In order to improve utility, ED
will change the page numbering of this form to a standard number
system (e.g., 1, 2, 3, ...).

This form collects data to evaluate
the performance of PWI grant recipients with repsect to their
compliance with evaluation standards mandated by Congress, to
enable RSA to meet annual statutorily mandated reporting
requirements, and to enable RSA to make determinations regarding
continued eligibility.
On behalf of this Federal agency, I certify that
