The child support enforcement agencies
and courts issues order/notices to employers to withhold income for
child support. Welfare reform requires the use of standardized
forms promulgated by the Secretary of HHS to ensure uniformity in
interstate case processing and ease the burden of compliance for
employers. The form will provide employers with the required
amounts to deduct child support payment from an
employee's/obligor's income. Respondents are private
companies/firms, Federal, State, local, and tribal governments who
employ noncustodial parents owing child support
obligations.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
