For future reference: only the cost burdens in excess of the wage value should be listed in answer to question 13 of the supporting statement. The wage value of the hour burden should be included in the answer to question 12 but not entered on form 83-I. The draft forms do not include a place for the expiration date of the OMB approval. The expiration date must be included on the forms to be used.
