with 5 CFR 1320, the information collected in forms CCC-36,
CCC-251, and CCC-252 is approved. However, to obtain approval for
the information collected in CCC-37, the agency must provide
information on the purpose, need, and practical utility of the
information collected. Furthermore, the agency must make an effort
to contact the parties affected by the collection to determine
whether the burden associated with the collection can be reduced.
The agency will provide OMB with the name, phone number, and
address of at least three of these contacts.

made jointly with another party, the other party must be

CCC-251 and CCC-252 for assignments or form CCC-37 for joint
