ethnicity questions (questions 6 and 7) to comply with the
Standards for the Classification of Federal Data on Race and
Ethnicity. Second, prior to fielding the pilot, please detail CDC's
plans for a more extensive network of sentinel surveillance sites,
including timetables for implementation, the extent to which CDC
plans to move toward a more structured and ongoing reporting
system, and the populations to be included.

The U.S. Centers for Disease Control
and Prevention (CDC) is applying for OMB approval for a form to
conduct a pilot project of sentinel surveillance for chronic liver
disease. Chronic liver disease, including chronic hepatitis and
cirrhosis, is an important cause of morbidity and mortality in the
United States.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
