The FCC will
only use this survey instrument on 6 entities. The proposed cover
letter (or a separate page) must include the PRA statement and the
OMB control number. The FCC has permission to not use the
expiration date if it dates each page of the survey. The FCC should
also ensure that respondents are provided with a telephone
number/email address of someone they can contact regarding
completion of the survey. OMB is also concerned that certain terms
used in the collection instrument might not be clearly defined. For
example -subscriber-weighted- may not be explained in the 1997
Cable Price Survey. The FCC shall review the instrument and ensure
that the respondents will clearly understand any terms of art.

The Cable Services Bureau seeks to
conduct a survey inquiry regarding the sources of programming costs
in the cable industry. This inquiry is to be conducted so that the
Commission, prior to the mandated end of rate regulation on March
31, 1999, cable programming service tiers, can better understand
the causes of recent increases in cable rates.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
