This collection is approved on the following conditions: CDC shall (1) revise question one of the surveys to allow for self- reporting of race and ethnicity in accordance with the revised standards on race and ethnicity; (2) revise the burden statement to meet the requirements under the Paperwork Reduction Act of 1995; and consider adding response categories to question 3(h). CDC shall send to OMB copies of the revisions prior to initiating the surveys. CDC's request to provide a $5 payment is not approved.
