This data
request will be used by all types of licenses contained in the
Universal License System. The FCC should consider allowing
licensees to respond to the request using email or facsimile -- and
include instructions to that effect in the correspondence directed
to the licensee. The FCC should also be clear whether this is a
voluntary or required obligation to respond. The language of the
Privacy Act/PRA statement seems to indicate that failure to respond
to the notice can result in cancellation of the license. This is
contrary to the PRA submission package. If response is mandatory,
the letter should so state. The letter and the PRA statement must
be consistent. The FCC shall provide OMB with the revised sample
letter correcting this item.

applications for modification if needed, to submit invalid licenses
for cancellation, and to keep the Commission informed of any
changes in mailing address.
