Terms of Clearance: Pursuant to 5 CFR 1320, OMB disapproves this requested collection because the design lacks practical utility for the purpose the agency has identified. The study design cannot be generalized to any larger population of persons who undergo prostate or colorectal cancer screening, and HHS's stated intent is to use the results of this survey to design policy and outreach with regard to future studies and initiatives involving these populations. Specific concerns with the proposed study include: 1) HHS has selected only three managed care sites in three States for a study intended to 'represent varied geographic areas and population groups' and for which recall validity 'should be generalizable to similar population groups' -- HHS has not demonstrated convincingly that the proposed design could support either contention; 2) the purpose of oversampling Blacks to understand overall trends for this group is based primarily on oversampling a single managed care plan in Georgia, not a sufficient basis to draw conclusions about overall trends; 3) A formal pretest of the survey instrument was not conducted on respondents who are similar to those who would complete an actual valid survey; and 4) HHS has not provided an assurance of confidentiality, which would be appropriate for collecting such sensitive information as prostate and colorectal cancer screening history. OMB's disapproval is based on a lack of demonstrated statistical validity, which would render the results unreliable for use by the agency. If HHS wishes to resubmit the package using a more generalizable design, or limiting the purpose of the package ONLY to a pilot study that would inform future more valid research, as well as addressing the other issues raised by OMB and discussed in HHS' memoranda to 6/25/98, 7/29/98, and 8/4/98, OMB would consider a new request. Assuming that this is done promptly, HHS need not republish its request for public comment in advance of OMB review, although the OMB resubmission should follow the standard PRA review procedures.

Abstract: The proposed data collection will obtain information on prostate and colorectal cancer screening rates and the validity of recall on these tests. 2120 members of three prepaid health plans will be interviewed by telephone, and medical records will be abstracted. This information will be used in program planning and health education.
