service delivery system that are served in CHIP based on the
Federal poverty income level categories and under age
categories.

Certain schedules of forms HCFA-21,
21B, 21P, 21.11A, and 21E will be used by States to report budget,
expenditures, and related statistical information required for
implementation of State Children's Health Insurance Programs, title
XXI of the Social Security Act (the Act), established by recently
enacted Balanced Budget Act of 1997 (BBA).
On behalf of this Federal agency, I certify that
